Patients & Visitors - Patient Identification


At Mackenzie Health, we are committed to patient safety. To ensure that we deliver excellent quality care to every patient, every time, we need to check at least two identifiers to confirm your identity at critical steps in the care journey.


Verifying the identity of patients using two independent identifiers is essential in order to ensure that our patients receive the care or procedure intended for them and avoid harmful incidents such as allergic reactions, discharge of clients to the wrong families, privacy breaches, medication errors and wrong-person procedures.


It Takes TwoTo ensure you receive the right care, we will check two of the following:

  • Home address
  • Date of birth
  • A recent photograph
  • Full name
  • Hospital identification number (Y or MRN number)

Information can be confirmed by the patient or a family member.


You can expect to be checked:

  • At registration
  • Before the initial assessment/examination
  • Before a procedure or treatment
  • Before specimen collection
  • Before medication administration
  • Before transfer to new area or procedure
  • When the meal tray is delivered
  • When a new member of the healthcare team becomes involved in your care
