The department of Pharmacy Services at Mackenzie Health serves all inpatient and outpatient care areas within the hospital. Our team of hospital pharmacists and pharmacy technicians provide you with safe and effective medications that your physicians prescribe.

 

Pharmacists are dedicated to helping you with your medication needs by performing medication assessments and monitoring to identify, prevent, and resolve actual and potential drug-related problems. Pharmacy technicians work to provide quality medications, prepared and delivered in a timely manner.

 

In addition, home visits to eligible patients are available from our team of Medication Management Support Services pharmacists for drug therapy assessments to ensure safe and effective transitions in medication use from hospital to home.

 

  • When coming into the hospital, ensure that you bring all of your medications in all forms possible e.g. bottles, lists, organizers, etc. The pharmacist would need this to begin your medication assessment. After this is complete, you will need to send your medications back home for safe keeping.
  • Ask questions about your medications during your stay in hospital. Your pharmacist, doctor or nurse can help clarify anything you may not understand.
  • On discharge from hospital, ensure that you obtain your prescriptions and a list of medications you were treated with in hospital. Share these with your community drug store and family physician very soon after you leave the hospital. These important documents will help them continue on with your care after discharge.
