Diabetes EducationWhat is Diabetes?

Diabetes is a chronic disease in which the body either cannot make insulin or cannot properly use the insulin it makes. This leads to high levels of glucose (sugar) in the blood. Having high blood sugars for a long time can damage organs, blood vessels and nerves. The body needs insulin to use glucose as an energy source. (www.diabetes.ca).

 

There are four types of diabetes:

• Type 1 (body does not make any insulin)

• Type 2 (body does not make enough insulin or doesn’t use it well enough)

• Gestational (body does not make enough insulin during pregnancy)

• Pre-diabetes (blood sugars are higher than normal but not high enough to be diabetes)

 

More than 9 million Canadians live with diabetes or prediabetes. Many don’t even know they have it. Chances are diabetes affects you or someone you know. No matter what type of diabetes you have, the Educators in the Diabetes Education Centre will help you make good choices as you manage your diabetes and prevent or delay complications.

 

How can the Diabetes Education Centre help you?

 

The Diabetes Education Centre is for adults with diabetes or pre-diabetes. We are here to help you make the best decisions for managing your diabetes.

 

Our Nurses and Dietitians are experts in diabetes and provide:

  • individual assessments
  • follow-up appointments
  • education and support to help you monitor and manage your blood sugars
  • diet education
  • medication adjustment
  • insulin pump therapy
  • group classes

Your visit can be in English or:

  • Cantonese
  • Urdu
  • French
  • Hindi
  • Italian
  • Mandarin
  • Punjabi

Classes are offered each month on many topics and languages. A six week series, Craving Change, is offered to help you feel more in control of your eating. 

 

Is there any cost?

 

Your visits and parking are free.

 

What can you expect to learn at the Diabetes Education Centre?

 

Some of the topics the nurse and dietitian will talk with you about are:

  • How to monitor your blood sugar
  • Ways to control your blood sugar
  • Healthy eating
  • Eating in special situations
  • How your diabetes medications work
  • How to manage your blood sugar during times of stress
  • What happens when you are sick
  • Diabetes and pregnancy
  • How to manage your diabetes when you  travel

 

How to book an appointment at the Diabetes Education Centre?

Physicians can refer their patients to this service by submitting the form by fax available on our website under “Staff and Physicians” and then “Online Forms” or by clicking on the form below. You may also call the number below to obtain a copy of the referral form.

When you come to your appointment, please bring your:

  • Health Card
  • Current medications, vitamins and any   herbal remedies (in their original    containers)
  • List of what you ate and drank for a full 3 days
  • Blood glucose meter and any records of
  • Your blood sugars
  • Doctor’s insulin order (if you are starting insulin)
  • We will call your family doctor to get your most recent diabetes lab work sent to us.

Monday 8:00 am - 8:00 pm

Tuesday – Friday 8:00 am - 4:30 pm

Saturday (one a month) 8:00 am - 4:00 pm
