When you leave the hospital, you may require in-home care. If so, you may discuss this with your nurse or the Clinical Practice Leader (CPL) who will contact the Community Care Access Centre (CCAC) Case Manager who will meet with you prior to discharge.

 

Physiotherapy, nursing, personal support such as bathing assistance, and intravenous therapy are only some of the in-home services provided to clients who require short-term care after surgery or because of illness or aging. You can also call the CCAC after you have returned to your home at 1-888-470-2222. A doctor’s referral is not required. CCAC services are free to eligible residents of Ontario with a valid Ontario Health Card.
