Planning for discharge begins on the first day of an admission to hospital. The healthcare team at Mackenzie Health works with patients and families to determine when patients are ready to go home.
Mackenzie Health, in collaboration with the Central Community Care Access Care Centre (CCAC), employs a new discharge philosophy called There’s No Place Like Home, which helps ensure that people are aware of viable options for care after leaving the hospital.
Evidence shows that a discharge home, even if it is to take the time to consider a future decision about long-term care, is the best solution. Because many seniors want to stay home as long as possible, the There's No Place Like Home discharge philosophy is meant to help them and their families understand their options for care. With the proper home care support, many seniors can return home to regain their health and maintain their independence.
CCAC Care Coordinators are available to provide patients with information and help them apply for support in their homes or admission to a long-term care home while in the hospital, as appropriate.
