About Us - About the Southwest York Region Health Link

Health Links is a new model of care that makes it easier for people who rely on our system the most to access health care in Ontario. Health Links is centered on one simple concept: physicians, specialists, hospitals, home care, long-term care and community services working together with patients, families and caregivers to better coordinate care.

As the lead organization for the Southwest York Region Health Link, Mackenzie Health has helped to foster the development of the Health Link and its collaboration with 20 community partners across the continuum of care to help make it easier for people who rely on the system the most, such as seniors and people with complex conditions, to access health care in our region.

Southwest York Region Health Link patients receive intensive support to help them stay healthier in their own home. They are connected to a dedicated Health Link Care Coordinator, participate in a case conference with their care team and contribute to the development of a coordinated care plan based on their unique goals.



Referring a patient to the Southwest York Region Health Link

Patients can be enrolled in the Southwest York Region Health Link through a referral from their family physician, following an admission to Mackenzie Health or by a referral from a community service provider.

Health Link patients are those individuals that have complex and high needs. They tend to be older, have multiple chronic conditions, and may have additional conditions or circumstances that impair daily functioning such as issues with mobility, limited caregiver support, challenges with Mental Health or Addictions, challenging social conditions, etc. These individuals have typically had a recent hospital admission and are at risk of returning to the hospital again in the near future.


Physicians and community service providers can refer patients into the Health Link by using the Central Community Care Access Centre (CCAC) Intake and Linking Referral Form.


In the open text area of the referral form, please indicate:

  • “SWYR Health Link”
  • The main reason why your patient would benefit from the care coordination provided by the Health Link.

Please attach the cumulative patient profile (CPP).


A Health Link Care Coordinator will contact you for insights on your patient's health history and to arrange for a case conference.
