Patients & Visitors - MyChart - Access Your Medical Record from Anywhere


We are upgrading Mackenzie Health’s MyChart on July 8, 2017.

MyChart is a secure health record information website for you to access and keep track of your personal health information recorded and provided by Mackenzie Health. It enables you to manage your health and puts your information in your hands.


If you currently have access to MyChart from Mackenzie Heath, please log-in as usual by copying and pasting the following link into your browser:


If you do not currently have access, please be aware we are unable to enroll new MyChart users at this time.

New accounts can be activated upon request after July 8, 2017.

Mackenzie Health’s current MyChart will be discontinued effective July 8, 2017.  Current MyChart users can expect an email from Mackenzie Health on or close to July 8, 2017 with instructions and a personalized PIN to re-activate their MyChart account once our upgrade is complete.


Why Is MyChart Changing?

Mackenzie Health is moving to a new, integrated electronic medical record (EMR) across the hospital and its community locations to enhance care for patients. The new EMR has a fully integrated patient portal (MyChart) which was built to work seamlessly with the new system.

Due to the volume of records being made available through the new system will provide you with access to you records from January 2015 and forward. This necessary change will enable us to enroll more patients to access their records.

Current MyChart users are encouraged to view their MyChart account to download any records they may require before July 7, 2017.

If you have MyChart access to your records from Sunnybrook Health Sciences Centre, these records will not be impacted. If you currently have access to MyChart from Mackenzie Heath, please log-in as usual below or copy and paste the following link into your browser:

Available July 8, 2017, Mackenzie Health will offer a new and improved online tool for patients to access their personal medical record from Mackenzie Health.


How Can You Register?

To request access to your records through Mackenzie Health’s MyChart email after July 8, 2017.


You can also provide your email at your next test or procedure. When you provide your email address, Mackenzie Health will send you a link and PIN to activate your personal MyChart account when you are ready.


New MyChart accounts cannot be activated nor PINs issued before July 8, 2017. 


Why Register For MyChart?


Effective July 8, 2017, you will be able to access your medical records directly from Mackenzie Health such as:

•      Labandtestresults

•      DiagnosticImagingresults(noimages)

•      View clinicalnotes on your care and treatment

•      Electronically check-in for appointments prior to arrival

•      Cancel appointments (various clinics)

•      List of your conditions or medical concerns diagnosed

•      Your list of your known allergies

•      Complete patientsurveys

•      View procedure instructions prior to appointments
