Patients & Visitors - MyChart - Access Your Medical Record from Anywhere

MyChartis a free online tool toaccess your medical record at Mackenzie Health from your personal device (phone, computer, tablet etc.). Log on to any computer, from anywhere, at any time, to stay on top of your health information including test results, appointments, summaries and medications from as early as January of 2015.

 

Signing Up for MyChart

You can sign up for MyChart in one of three ways:

1.)    When your appointment is being scheduled at Mackenzie Health, the clerk will ask whether or not you would like access to My Chart. If you agree, the clerk will ask for your email address. An email will be sent to the email address provided. Follow the link in the email and enter the PIN provided to get you started.

2.)    By providing your email address during registration at an upcoming appointment, you will be sent yourPINand instructionstosign up for MyChart.

3.)    Former patients can sign up at https://mychart.mackenziehealth.ca and follow the instructions.

Once you are signed up, log onto MyChart at: https://mychart.mackenziehealth.ca/ on your desktop computer or download MyChart to your mobile device at:
Google Play and Apple App Stores

 

Why Use MyChart?

With MyChart you can, view your medical information online, including:

  • Lab and test results,
  • Diagnostic Imaging results (no images),
  • Clinical notes on your care and treatment,
  • Access your list of conditions or medical concerns diagnosed at Mackenzie Health as well as your prescribed medications and known allergies, and
  • better manage your health information and appointments all in one place.
  • View your upcoming and past appointment record,
  • View preparation instructions related to your appointment,
  • Cancel appointments (available for various clinics), and
  • E Check-In for appointments 7 days prior to arrival.

E-Check In with MyChart

E-Check In is available to patients with a MyChart account and combines online pre-registration and check-in to get you to your appointment faster at outpatient clinics at Mackenzie Health.

 

Upon arrival on the day of an appointmentall patients register by visiting Patient Registration, located on the second floor of C-Wing, or designated Registration Area. Patients who have completed E-Check In will have fewer questions to answer or information to complete, getting them to their appointment faster.

 

You will receive an E–Check In notification to confirm and update registration information (address, phone number, emergency contact, family physician, health card version code/ expiry date) seven days prior to your appointment.

 

E-Check In can be completed up to the time of your appointment. However, if you would like to avoid a reminder phone or text message, complete E-Check In before 72 hours prior to your scheduled visit.

 

* For your security,  key identifiers such as name, etc. require notification to our health information services department, by contacting us at 905-883-1212 ext. 2004 (local to Richmond Hill) or 905-832-4554 ext. 2004 (local to Vaughan) to make these changes.

MyChart Security

We take great care in ensuring your personal health information is private and secure. MyChart is secured with similar encryption technologies as those used by major banks for online banking.

                                        

For your security, you create and control your MyChart password. 

 

Proxy Access for another Patients’ Record

If you are the legal guardian of a child under the age of 16, or if you have Power of Attorney for a patient, you can request proxy access to their MyChart account.
