Recognizing volume (fluid) overload as an area of concern for patients on dialysis, the Chronic Kidney Disease (CKD) team launched its first Back to Basics initiative: auscultation (listening to sounds of the chest) focusing particularly on breath sounds for pulmonary edema. The initiative was approached in three phases:
- An audit of nursing staff’s educational needs.
- A presentation (audio & visual) and a hands-on session in which nurses listened to each other’s breath sounds
- A post-audit to evaluate success and determine if another course of action was needed.
After B2B implementation, when patients were asked if a nurse had listened to your chest today—the response was an overwhelming YES!
90% improvement in nurses recognizing abnormal breath sounds and immediately addressing the fluid overload!
