Mackenzie Health’s Family Health Navigation Clinic provides continuity of care for our patients after discharge from any department in Mackenzie Health. Our team is dedicated to offering compassionate, comprehensive, timely and patient centered care with a shared goal of helping our patients to connect with a family doctor and/or other specialist needed within our community.

Referring a Patient

If you have visited Mackenzie Health’s Emergency Department, Urgent Care Centre, any clinic or have been admitted to an Inpatient Unit recently, your attending physician may refer you to our clinic if required.

If you do not receive an appointment within one week, please call us at 905-883-1212 ext. 2388 to arrange your next available booking.

Eligibility for referral to our clinic is determined by the attending physician at the hospital. Priority is given to patients who do not have a family doctor or primary care provider.

Cancellation & Rebooking Appointments

If you would like to cancel your appointment, you may cancel via online by using MyChart or by calling Mackenzie Health-patient scheduling office at 905-883-1212 ext. 2004. 

If you would like to rebook your appointment, please call our clinic directly at 905-883-1212 ext.2388. 

Please notify us 48 hours in advance if you need to cancel or reschedule your appointment. This allows us to accommodate other patients efficiently.  

Online convenience with MyChart

Through MyChart, you have online access to: 

  • Appointment calendar and details. 
  • Update your patient demographic at any time and e-check in seven days before your next appointment. 
  • Receive post-appointment clinical notes and test results. 

You can register for MyChart when scheduling an appointment or can sign up online at mackenziehealth.ca/mychart. 

What do I do when I arrive at the hospital?

Patients may park in the visitors parking lot, just north of the hospital, and enter through the D-wing entrance near the Emergency Department. Click here for more information on parking services. 

During peak hours, parking can take longer than expected. Please allow yourself extra time to accommodate this. Alternate options are to take public transit or be dropped off by a family member or friend. 

Check in at Patient Registration, located on Level 2, between the C-wing main lobby and the D-wing Emergency Department entrances where you will then be directed upstairs to our clinic.

How will our clinic help you find a family doctor in the community?  

Once you are referred to our clinic and consult with one of our in-hospital family doctors regarding any non-urgent medical concerns, we will provide you with a list of family doctors who are currently accepting new patients in the community and help you navigate to becoming a new patient with them. Our goal is to help you transition your care to a dedicated family doctor who can oversee all your medical needs. 

  

What is our navigation service?  

We support your transition to a permanent family doctor by offering temporary family doctor services at our clinic. Our Navigator will assist in guiding your care, ensuring follow-up on referrals from our physician and coordinate community support referrals as needed.
